APPLICANT - The questions we ask, and your associated answers, help us select the best puppy for you as well as the best family for the puppy. Please complete the following fields and click "submit." Most fields are required.

Do you have permission from the homeowner or landlord to have a puppy/dog for a pet?
Does your yard have a fenced in area for your puppy/dog?
Please list out all your current pets and include age and gender.
Please list the number of your current family, or live-ins, and include age and gender.
Please list any allergies that you, or person(s) living with you, have.

Will this aussiedoodle be an inside pet or an outside pet?
Have you previously trained a puppy?
Do you plan on providing obedience classes to your puppy?
Are you picking the puppy up or are you wanting to safely ship the puppy?
If the puppy is being safely shipped, what is the nearest airport to your home?
When will you be ready (what date) for obtaining a puppy?

Are you willing to spay or neuter your puppy by twelve months of age?
Are you considering breeding or becoming a breeder?
Are you willing to send follow-up photos and remain in touch with your puppy's breeder?
Are you willing to have your puppy/dog regularly groomed?
Are you willing to take care of, and ensure, your puppy/dog's ear health and keep them clean and free of hair and debris?
Are you willing to provide a testimonial/referral of our services and website for others to view?
Is it ok for us to post any photos of you and your puppy on our website and Facebook page?
Are you interested in an older puppy?
Do you agree if you ever have to rehome your puppy that we must be contacted first and given the right to locate a proper new home for the puppy? No refund will be provided to rehome your puppy.
Do you agree, that within two weeks of accepting/picking up your puppy, that you will take your puppy to your local licensed veterinarian in order to continue vaccinations and to have a professional health checkup?
